Integrative Orthomolecular Medicine Systems Consultation

Richard Z. Cheng, MD, PhD

A root-cause, systems-level consultation service for complex, unresolved, or chronic medical conditions requiring integrated interpretation of clinical history, laboratory data, metabolic function, environmental factors, and physiological regulation.

  • Editor-in-Chief, Orthomolecular Medicine News Service
  • Board Director, Riordan Clinic
  • Physician-scientist in IOM Systems Medicine

Consultations by Case Review

Due to limited availability and the complexity of many cases, prospective patients are asked to submit a brief case summary first. Not all cases may be accepted.

Consultation options: Initial consultation, comprehensive IOM consultation, and expert systems medicine case review.

What Is IOM Systems Medicine?

Modern chronic diseases are increasingly complex and often involve multiple interacting biological systems, including metabolism, mitochondrial function, immune regulation, environmental exposures, hormonal balance, inflammation, oxidative stress, and nutrient status.

Integrative Orthomolecular Medicine (IOM) Systems Medicine provides a root-cause, systems-level approach to understanding and addressing chronic health conditions. Rather than focusing only on isolated laboratory values or individual symptoms, an IOM consultation evaluates the patient’s overall biological terrain.

Integrated Review

  • Clinical history and timeline
  • Laboratory data and patterns
  • Metabolic and mitochondrial function
  • Environmental and toxicological factors
  • Nutritional, endocrine, and immune regulation

Who May Benefit

  • Complex or unexplained chronic illness
  • Multiple abnormal laboratory findings
  • Metabolic or mitochondrial dysfunction
  • Endocrine or immune dysregulation
  • Chronic fatigue, inflammation, or long-standing unresolved symptoms

The goal is to provide patients with a clear systems-level understanding of their condition and a structured, actionable path forward.

For Patients with Complex or Unresolved Medical Conditions

Many patients seeking consultation have already undergone years of medical evaluations, often seeing multiple specialists and receiving extensive laboratory testing, imaging studies, or genetic analysis.

In many situations, the challenge is not the lack of data, but the lack of an integrated systems-level interpretation of that data.

Dr. Cheng’s work in Integrative Orthomolecular Medicine (IOM) Systems Medicine focuses on analyzing complex clinical cases from a systems biology perspective and identifying physiological drivers that may not be apparent when laboratory results are evaluated in isolation.

For complicated or unresolved conditions, IOM systems analysis may help clarify:

Pre-Consultation Case Review

Prospective patients are asked to first submit a brief case summary. This preliminary review allows Dr. Cheng to determine whether an IOM Systems Medicine consultation would be appropriate and helpful.

Please include:

  • Age and primary diagnoses
  • Main symptoms and clinical timeline
  • Relevant laboratory reports or imaging summaries
  • Current medications, supplements, or therapies
  • Your main question or goal for consultation

This screening review does not include medical advice or clinical recommendations. If the case is suitable, scheduling information and consultation options will be provided.

Types of Consultation

1. Initial IOM Systems Consultation

$500
  • Review of basic medical history and available laboratory data
  • 45–60 minute video or phone consultation
  • Discussion of the systems medicine perspective
  • Initial recommendations for further evaluation, lifestyle modification, or nutritional strategies

Best for: initial guidance or a systems-level second opinion.

3. Expert Systems Medicine Case Review

$5,000+
  • Detailed review of extensive clinical data
  • Integration of metabolic, mitochondrial, immune, endocrine, and environmental factors
  • Written case analysis report
  • Systems biology interpretation of underlying mechanisms
  • Long-term strategic recommendations

Best for: highly complex cases requiring advanced expert review.

Payment and Refund Policy

All consultation fees are non-refundable.

Fees are paid to reserve professional time for case review, preparation, consultation, and/or written analysis. Once payment is made, the consultation process is considered initiated.

Because each case requires individualized preparation and allocation of professional time, fees are not refundable if the patient cancels, does not attend, decides not to proceed, or chooses not to use the recommendations provided.

If rescheduling is necessary, reasonable efforts will be made to accommodate a new appointment time, subject to availability.

Approach to Care

IOM Systems Medicine focuses on identifying and addressing root drivers of chronic illness, including:

The therapeutic goal is to restore the body’s optimal molecular environment, allowing normal physiological regulation and self-repair mechanisms to function more effectively.

Important Note

Consultations are provided as educational and medical advisory services. They do not replace care from a local physician or emergency medical services.

Implementation of any medical or therapeutic recommendations should be done in coordination with the patient’s primary healthcare providers.

For urgent or emergency medical problems, patients should seek immediate care from local emergency services.

Scheduling

To request a consultation, please submit a brief case summary by email. Selected cases appropriate for IOM systems consultation will be contacted regarding scheduling and next steps.
